Our Team

Dr. Casey Cain

PT, DPT

Casey Cain, PT, DPT is a Doctor of Physical Therapy with extensive experience in outpatient orthopedics, rehabilitation, and clinical leadership.

He provides individualized, patient-centered care with an emphasis on helping patients reduce pain, restore function, and return to the activities that matter most to them. His treatment approach combines patient education, therapeutic exercise, and hands-on techniques to help each patient achieve meaningful and lasting improvements.

Dr. Cain brings nearly a decade of experience as a physical therapist, including several years in clinical leadership. Prior to joining Lonestar Physical Therapy & Sports Performance, he served as a Regional Director overseeing multiple outpatient rehabilitation clinics while continuing to evaluate and treat orthopedic patients.

He earned both his Doctor of Physical Therapy and his undergraduate degree in Exercise and Sports Science from Texas Tech University. His additional training includes spinal manipulation, dry needling, instrument-assisted soft tissue mobilization, cupping, and manual therapy.

Casey is passionate about building strong relationships with his patients and helping them understand not only how to recover, but how to stay active and continue moving forward.

Outside of the clinic, Casey enjoys spending time with his family and being involved in the East Texas community.

Dr. Stephen Ford

PT, DPT, OCS, FAAOMPT

Stephen Ford, PT, DPT, OCS, FAAOMPT is a Fellowship-trained, Board-certified Doctor of Physical Therapy in Tyler, TX, and the founder of Lonestar Physical Therapy & Sports Performance.

He provides care for patients ages 12 and older with a guiding philosophy of empowering individuals to achieve a better tomorrow through better health today. His practice emphasizes patient-centered care, combining clinical expertise with compassion to help each person move, perform, and live at their best.

Dr. Ford is deeply committed to education and mentorship. He regularly teaches and mentors students, new graduates, and orthopedic residents, sharing his knowledge in advanced orthopedic manual therapy.

His background also includes advanced training in strength and conditioning as a Certified Strength & Conditioning Specialist, as well as additional specialization in dry needling, blood flow restriction therapy, headache and concussion management, and golf performance.

He remains actively involved in his professional organizations, including the American Physical Therapy Association and the American Academy of Orthopedic Manual Physical Therapists.

When he's not in the clinic, Dr. Ford enjoys time with his wife and children and can often be found on the golf course, where he combines his love for the game with his passion for movement and performance.

Dr. David Grubbs

PT, DPT, AT, OCS

David Grubbs, PT, DPT, AT, OCS is a Board Certified Doctor of Physical Therapy and licensed Athletic Trainer at Lonestar Physical Therapy & Sports Performance. He provides orthopedic care for patients ages 12 and older, helping individuals reduce pain, restore movement, and confidently return to the activities they enjoy.

Dr. Grubbs believes exceptional care begins with understanding each patient's goals and creating an individualized, evidence-based treatment plan. His approach combines hands-on manual therapy, therapeutic exercise, patient education, and progressive strength training to promote lasting recovery and performance. He lives by the philosophy, "If it doesn't challenge you, it won't change you."

Passionate about clinical excellence, Dr. Grubbs enjoys mentoring physical therapy students and has served as an instructor in a Physical Therapist Assistant program. His advanced training includes dry needling, manual therapy, and strength and conditioning. As a Clinical Athlete Powerlifting Coach, he has coached barbell athletes worldwide to improve performance and reduce injury risk.

Outside the clinic, David enjoys spending time with his wife and daughter, camping, visiting National Parks, strength training, and reading.

Dr. Hattie Trahan

PT, DPT

Hattie Trahan is a Doctor of Physical Therapy and a graduate of Texas Tech University Health Sciences Center.

She is passionate about helping patients move better, feel stronger, and return to the activities that are most important to them. Her approach to physical therapy emphasizes individualized, patient-centered care, combining evidence-based treatment with education and exercise to help patients build confidence in their movement and recovery.

Dr. Trahan enjoys working with a variety of orthopedic and musculoskeletal conditions and is committed to developing strong relationships with her patients throughout the rehabilitation process. She believes that understanding each patient's individual goals is an important part of creating a treatment plan that leads to meaningful, lasting results.

As she begins her career at Lonestar Physical Therapy & Sports Performance, Hattie is committed to continued professional growth and expanding her clinical expertise through mentorship, continuing education, and advanced training.

Outside of the clinic, Hattie enjoys spending time with family and friends and being part of the East Texas community.

Ayaz Ali 

PTA, CSCS

Ayaz Ali, PTA, CSCS, serves as Clinic Director and is a licensed Physical Therapist Assistant at Lonestar Physical Therapy & Sports Performance. . He graduated from the University of Texas at Austin with a Bachelor of Science in Human Nutrition and then completed the Physical Therapist Assistant Program at Tyler Junior College.

With a commitment to better serving the community and promoting a healthier lifestyle, Ayaz is also a Certified Personal Trainer with the National Academy of Sports Medicine Personal Training. He enjoys spending time with his patients in promoting physical activity, managing acute and chronic pain, and reducing fall risks with intensive balance programming.

In his free time, Ayaz enjoys spending time outdoors with his two dogs and traveling with his family and friends to new places.

Riley Borton

PTA

Riley Borton, PTA, is a licensed Physical Therapist Assistant at Lonestar Physical Therapy & Sports Performance. She earned her Bachelor's degree in Kinesiology from the University of Texas at Tyler and went on to complete the Physical Therapist Assistant program at Tyler Junior College.

Riley brings genuine enthusiasm and a strong passion for helping patients reach their goals. She values building meaningful relationships with those she treats and finds fulfillment in seeing each individual regain mobility, reduce pain, and return to the activities they love most. Her approach is centered on providing thoughtful, individualized care every step of the way.

Outside the clinic, Riley enjoys staying active by playing sports like pickleball and volleyball, curling up with a good book, spending time with her dog Hazel, and making memories with friends and family.
